Referrals can be made by healthcare providers, and other care providers including, social workers, front line workers, elders and community navigators.

Talk Tobacco’s referral program can be used on its own or as a positive adjunct to clinician intervention. The program eliminates the need for patients or clients to call Talk Tobacco. Instead, a Talk Tobacco Quit Coach will contact the them to provide counselling. 

You simply need to explain the program to your client or patient and encourage them to work with you to register for the service by completing and signing the referral form:

Upon receipt of a faxed consent form, the referral is entered into Talk Tobacco’s confidential database and a call is scheduled for a preferred time of day that the client or patient has identified.

Tobacco Cessation Specialists (Quit Coaches) provide information about the service, assess a client’s readiness to quit and offer services based on each individual client or patient’s needs.
